FAQ
What is physical therapy?
Physical therapy involves a structured, individualized treatment program that addresses a condition or injury through rehabilitation. The goal is to help the patient achieve a pain-free lifestyle and the ability to complete their daily activities. With exercise and hands-on techniques, we encourage the body’s natural healing mechanisms while preventing further injury. We provide each patient with a customized home exercise program to help further their progress.
Does a doctor need to refer me for physical therapy?
Because of Direct Access, a patient can receive treatment from a licensed physical therapist without a referral from a doctor. At Ascend Physical Therapy, we offer same-day appointments and will communicate with your doctor for you.

Will insurance cover physical therapy?
Our office accepts most insurances, including Medicare. We will evaluate and explain your insurance benefits before your first visit. We do all billing in-house, so we can answer any questions you may have about your benefits. If you don’t have insurance or if your insurance doesn’t cover physical therapy, we also have self-pay rates.
How long does a session last?
Your first visit will be an initial evaluation, during which the physical therapist will evaluate, diagnose, and create a customized treatment plan. This appointment takes about one hour. Each follow-up appointment takes about 45 minutes. We are committed to accommodating your schedule to make physical therapy convenient for you.
Patient Forms
To save time in the office before your first appointment, you can download, print, and fill out your patient forms beforehand and bring them with you when you visit.
